How to fill in the questionnaire:
Dear customer, we kindly ask you to indicate what importance you attribute to the various aspects of the service indicated below, in other words, tell us how interested you are in each individual topic. At the same time, we ask you to indicate the level of satisfaction or perceived quality the various aspects of the service have managed to generate.
Any suggestions you have (Section VI) will be very useful to us: do not hesitate to share your views!
The scale applied is 1 – 9 (in which 1 indicates the lowest level of satisfaction and 9 indicates the highest level).
Select 0 if the service has not been used by you or if the item on the questionnaire is not relevant to the service you have used.
